Pompe Disease is a rare, inherited disorder characterized by severe, progressive muscle weakness and respiratory impairment. It is caused by mutations in the gene that encodes an enzyme called acid alpha-glucosidase, also known as GAA, which is needed by the body to break down glycogen – a stored form of sugar used for energy. Pompe Disease affects approximately one in every 40,000 births.

The goal of gene therapy for Pompe disease is long-term expression of acid alpha glucosidase after administration of a vector carrying the GAA gene. Increased levels of acid alpha-glucosidase are expected to produce improvement in symptoms related to the disease. Multiple studies have demonstrated encouraging results for the treatment of Pompe after delivery of the acid alpha-glucosidase gene using adeno-associated virus.

Systemic administration of adeno-associated virus to deliver acid alpha-glucosidase in GAA-deficient mice demonstrated significant increases of GAA activity in cardiac and skeletal muscle, which led to improved diaphragm contractile strength, reduced cardiac glycogen, reduced left ventricular mass, and improved ejection fraction. In addition, a recent study in a mouse model of Pompe disease compared the effects of a single systemic injection of AT982 (AAV-GAA) to bi-monthly injections of recombinant human GAA (ERT) over a three-month period. Improvements in diaphragmatic contractile and cardiac function were observed in both treatment groups at three months post-injection compared with untreated animals, while breathing frequency and expiratory time were significantly improved in AAV-GAA treated animals but not in ERT treated animals. In addition, glycogen deposition was significantly elevated in untreated and ERT animals, but not in AAV-GAA treated animals at three months post treatment. ポンペ病は重症で、進行性の筋力低下、呼吸機能障害から特徴づけられる希少な遺伝子疾患です。この病気は酸性αグルコシダーゼと呼ばれる酵素がコードされている遺伝子(別名GAA)の変異によって引き起こされます。この酵素は、体内でエネルギーを貯蔵するための糖である、グリコーゲンを分解するために必要です。ポンペ病は新生児の約4万人に1人の割合で発症します。

ポンペ病に対する遺伝子治療の目標は、GAAを運ぶベクターの投与後、長期に渡って酸性αグルコシダーゼが発現することです。酸性αグルコシダーゼの濃度の増加は病気に関連した症状の改善をもたらすと期待されます。複数の研究では、アデノ随伴ウイルスを用いた酸性αグルコシダーゼ遺伝子の輸送の後、ポンペ病の治療にとって励みになる結果を示してきました。

GAA欠損マウスにおいて、酸性αグルコシダーゼを運ぶアデノ随伴ウイルスの全身投与は、心筋と骨格筋でのGAA活性の著しい増加を示しました。それは横隔膜の収縮強度の増加、心臓のグリコーゲンの減少、左心室の体積の減少、駆出率の増加を示しました。(Byrne et al, Human Molecular Genetics, 2011, Vol 20, Review Issue 1)

​さらに、最近のポンペ病マウスモデルでの研究では、三ヶ月間に渡って、AT982(AVV-GAA)の一度の全身投与の効果とヒト組換えGAAの隔月投与(ERT)の効果を比較しました。どちらの治療群でも未治療群と比較して、投与の三ヶ月後には横隔膜収縮と心機能の改善が見られました。しかしAAV-GAA治療群では呼吸頻度と呼気時間が著しく改善しましたが、ERT治療群では改善しませんでした。加えて、未治療群とERT治療群ではグリコーゲンの蓄積が著しく増えましたが、AAV-GAA治療群では治療後三ヶ月では増えませんでした。(Falk, et. al., Molecular Therapy – Methods and Clinical Development (2015) 2, 15007)


What is gene therapy? 遺伝子治療とは何ですか?

Gene therapy is a form of treatment involving the administration of genetic material to patients for the purpose of providing a clinical benefit. It is designed to restore the presence of normal proteins to treat a patient’s disease. Gene therapy works by introducing a functional copy of a gene into the cells that contain the defective version of that gene, allowing the cells to produce the normal protein. Gene therapy products are composed of “vectors,” which are typically viruses that have been engineered to deliver a therapeutic gene to a patient’s cells. Gene therapy vectors direct a patient’s cells to make a new protein that substitutes for a defective protein in the patient. Once the cells are able to produce the correct protein, gene therapy may ultimately lead to improvement in the patient’s symptoms.

​遺伝子治療は患者への遺伝物質の投与を伴う治療の一つの形です。臨床的な利益の提供を目的としています。その遺伝物質は患者の病気を治療するために正常なタンパク質が存在し直すように設計されています。遺伝子治療は機能的な遺伝子のコピーを欠損遺伝子をもつ細胞へ導入することによって行なわれます。これにより細胞が正常なタンパク質を産生するようになります。遺伝子治療の製品は「ベクター」が含まれています。これは一般的には治療のため遺伝子を患者の細胞へ輸送するウイルスです。遺伝子治療ベクターは患者の細胞へ、欠損タンパク質に取って代わる新たなタンパク質を作るように命令します。一度細胞が正常なタンパク質を産生することができれば、遺伝子治療が患者の症状を改善する最後の治療につながるかもしれません。


What is adeno-associated virus? アデノ随伴ウイルスとは何ですか?

Adeno-associated virus (AAV) is one of the most frequently used viral vectors for gene therapy. A small virus that is common in humans, AAV is not known to cause disease. When engineered as a vector for gene therapy, AAV has a known clinical track record, is nonpathogenic and is already approved for use in one product. It is also currently being investigated in multiple clinical trials. Given the promise of engineered AAV vectors for one time administration of gene therapy, and their ability to harness the cell’s own systems to encourage normal protein production, Audentes is exploring the use of adeno-associated virus to address diseases caused by a single gene modification (also known as monogenic diseases) such as X-Linked Myotubular Myopathy (XLMTM), Crigler-Najjar Syndrome (CN), CASQ2-related Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT) and Pompe disease.
